A patient picks a speciality, a doctor and a time without getting through to reception. The booking reaches your Telegram, and the patient sees their visits in the app.
The screens are already wired together: buttons lead where they should, forms send, lists are drawn from a table.
A layout of the screens rather than a screenshot of a finished app: how it looks depends on your colours, type and content.

What gets changed first
Specialities, doctors and prices are a table, with no layout to edit.
Booking fields are yours: reason for the visit, first or follow-up.
Hours and locations are rows, updated from a phone.

Questions about this template
Can medical details be collected?
The form takes whatever you put in it, but responsibility for personal and medical data rests with the app owner. Sensitive detail is better asked at the appointment than in a request.
Will a patient see their visits?
Yes, when your own bot is connected: a visitor’s records are identified by its signature and survive a reload.
How is an appointment moved or cancelled?
The booking row is edited in the table from a phone; the front desk does it between appointments, with no editor open.
